PRESCRIPTIONS & CONTROLLED SUBSTANCES AGREEMENT

In our efforts to provide comprehensive mental health services, Meaningful Change: Clinical & Consultation Psychology utilizes pharmacotherapy and medication management services in treating a range of psychiatric disorders. Psychiatric services at this practice are provided by a Physician Assistant who specializes in psychiatry. Since psychiatric medication and controlled substances (Anticonvulsants, Benzodiazepines, Sedative-Hypnotics, and Stimulants) require more responsibility from both patient and prescriber, signing a "Prescription & Controlled Substance Agreement" will be a mandatory part of the intake process.

SPECIAL POLICIES FOR THE USE OF STIMULANTS IN THE TREATMENT OF ADHD

It will be the policy of Meaningful Change: Clinical & Consultation Psychology and its medications prescribers that the “stimulant” class of medication will only be utilized in the treatment of attention deficit/hyperactivity disorder (AD/HD) when the patient has undergone formal testing and diagnostic examination by a licensed doctoral-level clinical psychologist, school psychologist, or neuropsychologist. Ultimately however, the appropriateness of the diagnosis, and suitable prescription(s), is left to the medical judgement of the prescriber.
